Boise State University will assist qualified students in receiving equal access to print course materials. After reading the Alternate Text Format Agreement, please complete the form below to request your books in Alternate format

Please be advised: Students requesting alternate format for classes must have met with a Disability Specialist at the Disability Resource Center and have provided the necessary documentation BEFORE request may be filled.

I agree that I have read and understand the DRC Alternative Text Format services guidelines. I agree to purchase my required textbooks that I am requesting in alternate format from the DRC as a new textbook or in like new condition. I will retain the textbook for the duration of time I have the alternate format version. I understand that the alternate format is for my sole use and I will not copy, distribute, or share the alternate format. I understand by not complying with these guidelines this is a violation of the Boise State University Student Code of Conduct.
